Social Worker

GENERAL PURPOSE OF THE JOB This position provides guidance assessments support and information to patients and their families to maximize the psychosocial functioning and adjustment of the patient in chronic kidney disease.
ESSENTIAL DUTIES AND RESPONSIBILITIES The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive. • Independently handles psychosocial related assessments and recommendations. Effectively and efficiently handles unique or very difficult situations • Independently conducts initial patient psychosocial assessment within 30 days of admission and conducts an independent reassessment within 90 days after the initial assessment. • Identifies and develops treatment and psychosocial goals for an initial plan of care and updates the plan of care as it needs to be modified• Identifies assesses and documents psychosocial evaluations quarterly and annually evaluates and monitors ongoing psychosocial issues and patient goals • Conducts reviews and assesses quality of life survey results develops patient goal setting completes appropriate follow up to analyze patients' plan to improve scores and quality of life. To be performed periodically annually at minimum. • Communicates with interdisciplinary team regarding patient psychosocial issues affecting treatment and wellbeing • Independently analyzes and provides information and facilitates referrals to community and private agencies patients’ charities and Saudi Center for Organ Transplan SCOT and federal agencies based on psychosocial needs• Counsels new and current patients and families on treatment options and modalities e.g. transplantation PD provides support information and assistance in treatment planning provides assistance to patient and family in adjusting to dialysis and altered lifestyle provides resource information • Conceptualizes problems interventions and the evaluation of interventions with respect to knowledge of human behavior personality development and social environment. • Complies with the rules and regulations of the Kingdom • Works independent as a Nephrology Social Worker • Works collaboratively with the Clinical Services on company initiatives • Promotes and may oversee the standardization of Social Worker documentation tools initialannual assessments quarterly notes and patient care plans • Leads team discussions related to patient engagement and support of patient’s choice and right to self determination • May serve as Patient Experience of Care Lead
Skills•Licensed or registered in the Saudi Council as required by the Kingdom’s regulations
•Degree in Social Work MSW
•Basic computer skills in MS Word Excel PowerPoint and Outlook as well as functional proficiency with DaVita specific applications

LANGUAGE SKILLS
Arabic and English language skills required.
